{Perthes Disease} FAQ's

My post about Nate's Perthes was long and drawn out, so I thought I'd share some general information about Perthes Disease here and as we find out. We are always learning more with each doctor's visit to Shriner's Hospital in St. Louis.

So what is Perthes?

Perthes is when the blood flow stops going to the top of the hip and causes the bone to die. There are stages to this as shown below.

Perthes is patient specific, meaning that each and every case is slightly different. Age of patient, stage they are in when diagnosed, etc. can all play an important part. Time and length of disease can also vary, as it's about the bone dying and being regrown.

Doctors have differences on treatment. Some are wait and see with restricted activity, some are straight to surgery. This can vary depending on patient too.

Long term effects and complications can arise, but not always. This isn't a "fix it now and be done forever" issue.

Random Info:
We've been told some small tidbits of information in appointments and as I hear more, I'll share here.

One of the biggest things is range of motion. This was mentioned at Nate's first post-op check-up. We asked about when you know he's ready to not have the brace and the doctor mentioned range of motion. So there's no time specific date for the brace, although Nate will be in for the rest of the school year - roughly 5-6 months.

Nate the Great

This will be a page I update as I find out more, helpful information. From now on though, new blog posts will deal specifically with Nate and his growth and updates. We have insurance but there's a copay and a percentage left for me to pay.

{Life Lately} Nate and Perthes Disease

It's about time for an update, so let's just jump right in and learn about what's been taking up my time, energy, money, stress levels and so much more. Edited to add: this is long and I'm trying to break it up for updates. 

In the beginning of the year, Nate, my son, started to limp. At first, we blamed the planter's wart on his foot and then the blister that the medicine made didn't help. He had the medicine put on the last week of March. The blister and healing after the blister took most of April. In May, the limping was still bad enough that we finally were able to be seen by an orthopedic doctor. One of the nurses in our local hospital's ortho unit, noticed his hips moved odd and stiff, so we had x-rays done right then. She brought up something that we had never heard about (and neither have many others) and that's Perthes Disease. It's technically Legg-Calve-Perthes Disease.

I hate the word disease, so we'll shorten it to Perthes.

So what is Perthes?

Perthes is a random, rare issue that is caused when the blood stops going to the top of the femoral head. This can happen for unknown reasons and is not necessarily trauma based. Nate wasn't in an accident, he wasn't hurt in any obvious way. He was a very active 5 year old, preschool loving, energetic boy with no previous health issues.

The doctors told us to follow the "restricted activity" method and so we pulled him from baseball and kept him from running and jumping as anyone can control an active 5 year old. He could still swim, which was great because our family's love of water and how it's a great physical activity for hip problems.

But Perthes and the lack of blood flow to the femoral head then causes the bone to die. This process can take a long time and is called Necrosis (dying bone).

The second stage is fragmentation, which is where both of Nate's hips currently are.

In November, after months and months of therapy (physical therapy and water therapy), I went with my gut feeling (aka Mom's Intuition) and called Shriner's Hospital in St. Louis for a second opinion. I took Nate up by myself, thinking they would be saying something similar to what our local doctors were saying. I was wrong! He was told, by Dr. Schoenecker, that he shouldn't be walking at all and that he recommends surgery. At first I thought "really?" then realized that I went to Shriner's for a reason - they see Perthes every day, multiple times a day. They've seen it in all stages. Our local doctor has seen maybe 1-2 cases in their career. Dr. S. is also known around the United States as a leader in Perthes and Perthes treatment. As much as I hated the word surgery, I trusted this man to know what he's talking about. Dr. S also mentioned that he had it in both hips, which is even more rare. Our local doctors knew this (it was in paperwork I printed off after getting our Shriner's appointment) but never told us this.

One week before Thanksgiving, Nate went in for surgery. He came out of it wearing a Petrie Cast in bright red (his favorite color). He had a abductor tenotomy, where a tendon is cut. They were able to push one of the hips back into socket without a lot of push back and hopefully the cast will help do its job. You can read more of the treatment here, if interested.

After Christmas, Nate gets his cast off and he will get his brace. It'll still be an A frame type shape and will be worn 20 out of 24 hours of the day. Which means he'll have time for therapy and baths. I'm hoping that he will have aqua therapy again, which helps build strength.


{Nate the Great} Picture Time

Being 2 is hard. It's been hard for the boy and it's hard for this momma to get a good picture. He's all about "no take picture!" 



The below photo cracks me up. He was mad at me after I yelled at him this weekend because he almost walked into a thorn tree. The thorns were 3" long and EVERYWHERE. The downside of country living at my parents' house, I guess. So he refused to follow us to the other side of their smaller pond. And there he sat, mad as can be, until he got over it and came to see what we were doing. In about a month, these paths will be different and so much better to walk on - as in no bugs and hopefully no more poison ivy!!! 

{Nate the Great} One Year Old!


Last week, this little guy turned one.

He's a handful in all the good ways. He's all boy, all energy, all the time.

He's in 18-24 month clothes, size 5 shoes, and size 3 diapers.

He's around 26 pounds. His yearly check up has been pushed back until Thanksgiving due to us being off since the beginning. But, he had a "sick child" visit last week due a bad diaper rash so we got him weighed. Not sure on the height but he's tall, at least to me.

He's had his third haircut recently. It's a very blonde strawberry blonde, so it'll be interesting to see where it goes. 

His eyes are still blue. I love my blue eyed babies! 

He's been walking for over a month now and is excellent at it, except when he tripped on the pumpkin vine at the Pumpkin Patch and bit into a pumpkin. 

He's ornery but I love him. He'll be the most stubborn kid I know,  I'm just positive.

He's definitely the best thing to our family since Miss Lily. He completes us and I can't wait to see what the next year (and more!) have in store for us.

{Life} Messy Kids



"Hi, my name is Nate and I love food." 

Can I teach him that?!? While giving a person a hand shake (and a strong one too!). 

And yes, that's a blueberry piece on Lily's tooth. She's fully aware, thought it was funny, and wanted photos of it. She then got sad it finally came off. True story, I can't make this stuff up.

Nate's issue? His momma dumped out a bag of freeze dried fruits on his tray and half the bag was dust. He thought it was great. The white stuff is cottage cheese. And there's some puffs in that mix too. He has a well rounded meal.

And just because... here's Lily covered with freeze dried strawberry powder from February 2011...



{Nate the Great} Foods at 7 Months

I asked friends and family on ideas for foods for babies on Facebook, so I figured I'd post here too. These are the foods Nate has had...these are, of course, cut up into bite size pieces when necessary...

Chicken, ham, hamburger, hot dog
Lima beans, green beans and refried beans
Bananas, apples, strawberries, melon, pears
Spinach, pickled beets, potato wedges, avocado
Fries (yeah, not the best but the kid steals your food)
Rice
Bread, rolls, chips, crackers
Eggs - scrambled, egg whites only, and hard boiled
Puffs, baby "cookies", teething biscuits
All types of canned baby food
Cottage cheese, cheese, yogurt, ice cream (just a little!)

I'm not sure that's all but it's a pretty big list for a 7 month old. He's less picky than his 5 year old sister!

{Life} Catch Up

Oh my goodness.... Summer is already here (as in I'm done with work until August) and things are moving too fast. 

Nate is 7 months old and had his first hair cut. It makes him look so much older. He goes for his wellness check Friday, so we can finally see how big he is!


And then there's the princess. My oh my. She's my little fish and would spend all day, everyday at the pool. Lucky for us, my parents have a new one. Plus, I won passes for our local pool. And, Lily now goes to the local pool with preschool three times a week. Talk about happiness! Yesterday was their first visit and she slept for over 12 hours last night. 


And back to the busy bee... I get to stay home with him this summer. It lowers my productivity by about 90% but I wouldn't have it any other way. He is my little animal lover. 

{Lily} Yearly Photos

5 years and 5 days ago was my third wedding anniversary with Doug and I was due any minute with Lily. She came three days later but I had gone to town to walk in a beautiful city gardens right in the middle of town. I figured it'd be a safe spot, just a few miles from my doctor, if anything were to happen. Nothing did other than some photos of flowers. However, I realized that I could bring Miss Lily to do yearly photos and it would provide a great back drop. At some point in time, probably after the invention of Pinterest, I came up with the numbered shirt idea. I kept it simple - white shirt, pink number with white polka dots. Her grandmother made them (and has every year!).

Here she is over the years... 


I love watching her change but the background stays almost the same. I'm planning on doing the same thing for Nate but maybe in a more "manly" setting... a state park possibly. His will happen in the fall so the colors would be great!

This year was obviously a little different as Nate came with us. I brought the stroller to keep him contained (he's not walking or crawling yet but it helped a lot to keep him somewhere safe and out of reach of everything!). As we got to the gardens, it was about to rain but we got in and out before it came! It did bring some cooler temps though, so luckily we had a matching jacket in the car. The girl loves her pink! 

There is a series of photos with these two and I just adore them. They make my heart melt. She loves him, he adores her. Two beautiful babies if I say so myself! 

And as Lily was getting fed up with the show, I put Nate down and took a few of him. It looks like he's showing off these flowers... which I want by the way... he also tried to eat them. Good thing they aren't poisonous! 

{Nate the Great} 7 Months

He's 7 months old already!! 

Yes, I'm fully aware that I missed this at 5 and 6 months... life is busy! But we're back on track and here's to hoping I continue it. It's not like he's not photographed... I just didn't do it in his chair with his bear. 

A little comparison... not many similarities! I think he looks a lot like her NOW but as a baby, she had different characteristics. 


And the last comparison...

Nate at 7 Months:
Two teeth, working on the uppers I think (Lily had at least three teeth as shown in the photo).
He's wearing 9-12 month clothing, sometimes 12-18 depending on brand and how they size things.
He's super long and big! I'm guessing between 20-25 lbs, we should know in a week or two when we see the doctor.
His hair is out of this world. I trimmed up the back "mullet" and now it's super obvious that he needs it professionally done. Lily was way older than 7 months before I cut her hair! His hair hangs in his eyes. There is a slight curl in it but not like Lily's.
He's a great eater and loves everything he's tried except for refried beans. 
He had one major poop explosion and played in it. My mom was the wonderful woman who had it happen to her and had to clean it up. 
He's not the best sleeper but is getting better at naps. Here's to hoping! 
He smiles all the time. Seriously! Even when he's sad, you can get him to smile. Today it was a smile/laugh/cry episode while tired.
He doesn't have stranger anxiety yet. 
He's a mover. He (kinda) crawls and rolls all over the floor. 
He loves to stand up and be held up. I have a feeling he'll be running soon. 
He adores animals and gets giddy when Rocky comes into the room.
He loves his big sister and will stare at her all the time. 
The red spot on his nose is still there but not any bigger. I hope it'll fade at some point or else the doctors will deal with it. 

{Life} Sleeping Babies



Lily came home from preschool just as tired as can be on Tuesday. The poor girl had such a busy weekend with her preschool graduation and birthday, that come Tuesday, she was out before 6. 


And then Nate...he's 7 months old and fights sleep just like big sis. He does, however, love to sleep with a blanket covering his face. 

{Boy vs. Girl} 4 months


Nate is winning in the hair department! 

At 4 months old..
Both have tried rice cereal, sweet potatoes (as Lily is showing off) and other baby foods.
Lily was the better sleeper, Nate is the better eater.
Both are big (18 lbs).
Both have blue eyes, the doctor told us Nate will be keeping his. Nate's are darker.
Lily has Doug's face shape (rounder), Nate has mine (longer).
Nate has bigger ears (they look giant here!), Lily has Doug's ears.
Very similar noses, Nate's looks wider.
Nate is in 6 month clothing (can even go up to 9 months). I'm thinking Lily was the same.
Nate rolled over front to back and back to front this weekend. I think Lily could do it too.
Both are able to push up on their hands.
Both love being hold and walked around the house.

{Nate the Great} 4 Months


At 4 months, Nate is hitting all of his milestones and then some. He is the happiest little guy and except for the drooling (never ending, as you can tell from his red cheeks and chin), he has no problems. The little spot on his nose, per the doctor, should clear up by 2. It hasn't gotten bigger so I'm hoping it'll fade away soon. Speaking of the doctor, she said he was perfect and that he's now 18 pounds, doubling his birth weight. He's in the 85-90% for his height and weight and his head is around 100%. He's a little smaller than Lily was at this age but that's ok! 

I love the picture here because he's smiling at Lily. He LOVES her and yes, that deserves the all caps. She can get him to smile and laugh quicker than anyone else. It's great. 

You'll notice the giant text in the corner of the pic.. and yes, I purposely put it slightly over his head. For family members that want a copy of the picture, let me know. I did this due to not wanting people stealing my kids' photos (a fear I have). This blog isn't famous and maybe 10 people read it, but you never know! However, I do want to post and share so people know what's going on and so I'll still do some things... 

{Nate the Great} Snow

Don't we all just hate the snow now? This isn't our first snowfall this year and I have a feeling that it won't be our last. I'm crossing my fingers though, 'cause I am sick of it! The photo below shows that it's roughly 8" of snow. That's on top of a nice layer of ice too.

{Nate the Great} Three Months



Nate is officially 3 months old... ok, so it was January 25... I'm a little late. Maybe on his 4 month shot, I'll be on time. I took the photos on time, but getting around to a post hasn't been the easiest. But now we're going into day 3 of snow days, so I have some free time.


The above photo shows off his cheeks. The below photo has his tongue sticking out. To bad my camera and I aren't working well together (and some super strong back lighting doesn't help but that was my fault).  


The photo below shows "his look". Lily had a similar one, I'll have to find it and compare. 

This is one of my favorite photos of him. He's not a huge fan of smiling for the camera because he gets kind of shy in front of it (or mesmerized is almost more like it). But this one I had to work for, bubbles and all! 

And then I tried to lay him down in his bed for more photos with the bear and he was NOT having it! 


As for what he's doing:

"Talking" a lot
LOVES to stand up while we hold him. Many times he fusses just so we stand him up and he instantly smiles.
Loves watching people, especially Lily.
Has tried both rice cereal and sweet potatoes, but both were just for fun. He's still all bottle for food.
Kicks a lot with his legs
Pushes up easily while on belly time
Holds his head up with no problem
Will lean back while you hold him, giving your arm a work out (and making sure you pay attention while walking through door ways!)
Still has the little red spot on his nose
Smiles all the time

Physically:
We aren't sure how much he weighs or how long is he. 
He's in size 2 diapers.
He's in 3-6 month clothing but can fit in some 3 and some 6, depending on what brand.

He's easily one of the happiest, best babies around. He fusses some at night or when he's tired, and if he's hungry, but not at much else. He loves cuddles and being talked to. We are definitely blessed with this little guy! 

{Nate the Great} 2 Months

I am so behind! Nate's 2 months fell on Christmas and it's been crazy ever since as we try to adjust to both of us working full time, Lily in preschool and life as a family of 4 still. Mainly I just can't get into a groove, but I feel that is due to my now THREE month old that hasn't gotten a schedule yet either. He is always growing and ALWAYS eating! I swear he'll out eat Lily soon.


Here are Nate's two month shots. After reviewing Lily's two month shots, I think I may move back to taking some in the crib to show any difference in size by measuring against it. We'll see. I need to take his three month shots now! 









{Life} Photo Dump

Wow! How are we already almost half way through January? 





(All above photos were taken Christmas Eve at my parents' farm.) 


Last week, as everyone was aware of, we had horrible weather and it postponed the beginning of our Spring Semester by two days. So, instead of starting on Monday, we started Wednesday. Although I miss spending all day with Nate the Great, I know he's in good hands with my mom. I also missed some of my coworkers and being around adults through the day. I'm still learning my new students as I have about 40-50 new faces. I have roughly 100 students that I see daily but I know most of them. My 6th graders (I teach 6-8 for those that don't know) are new to the building so I may recognize them from last semester but I don't have all of their names down yet. Getting there!







Photos above include Christmas Eve at my parents' house and a later celebration of Christmas at my in-laws' house. The above photo was a challenge to get! Four kids, 4 and under! Lily and Will are 3 months apart (Lily being older) and Nate and Kyla are 3 months apart (Kyla being older). This Christmas was calm compared to what future years will hold! 

Below: Can you believe Nate is three months YOUNGER than her?! 


As for the rest of life, it seems to be flying by! I wanted to get some photos uploaded since Christmas but between spending extra time with our family and Lily (we kept her home from daycare for two weeks), we've been busy with being lazy and enjoying each others' company. 






So...instead of random posts about individual things, this is a "catch everyone up to date" post and I'll hopefully get on a schedule at some point. I also have a blog for my classroom and I want to work on it, getting pictures and information up on it, so hopefully I can do both. 

Doug and I are both working full time, Doug's doing extra coaching after school (and that'll continue until May), Lily is loving preschool and being with friends, and my parents are watching Nate.
